Endophthalmotis Vitrectomy Study (EVS) 1995 ⏩
N=420, 9m
Study question: S/P cataract. Tap vs PPV. All got Vancomycin 1mg/0.1ml and Amicacin 0.4mg /0.1 ml + Subconj Vancomycin Fortum and Dexacort. One arm got IV Fortum and Amicacin. Study Results: Presentation Mean 6-10d, median 6d. 22% 2-6w. 70% positive culture, 94% gram positive, 70% Coagulase(-) 10% staph aureus. Systemic antibiotics had no benefit on local use. When VA at presentation is HM or better, there was no difference in visual outcome between PPV and T&I. VA of LP VIT produced 33% 20/40 or better (vs 11%) VA 20/100 or better 56% vs 30%. A 50% decrease in the frequency of severe visual loss with PPV (20% vs 47%) over TAP.

Study question: Intra-cameral cefuroxime 1 mg VS topical levofloxacin for prophylaxis of post-operative endophthalmitis. Study results: Found A decrease from 0.34% to 0.07% in patients given IC cefuroxime 1 mg VS topical levofloxacin (Odds ratio of 5.59). Note higher than expected rates in the topical tx gruop. See Cochrane review on this issue. Found a 5.88-fold increase in risk with the use of clear corneal incisions instead of scleral tunnels.
