Polypoidal choroidal vasculopathy (PCV)

Efficacy and safety of verteporfin photodynamic therapy in combination with ranibizumab or alone versus ranibizumab monotherapy in patients with symptomatic macular polypoidal choroidal vasculopathy (EVEREST study), 2012 
n=61, 6mo
Study question: Ranibizumab vs PDT vs combination for PCV. A randomized clinical trial.
Results: Ranibizuab alone or Combination therapy was significantly superior in complete polyp regression (77%, 71% vs 28%). BCVA improved but study was not powered to compare (10.9 gained letters for combination, 9.2 ranibizumab, 7.5 PDT). See below EVEREST 2 published in 2017. $ Novartis.


Efficacy and Safety of Ranibizumab With or Without Verteporfin Photodynamic Therapy for Polypoidal Choroidal Vasculopathy (EVEREST 2), 2017  and  
n=322 Asian only, 1y (on going as of 2017)
Planned to overcome EVEREST 1 downsides (see above). Study question: 0.5 Ranibizumab with or w/o PDT for PCV. 3 inj then PRN. RCT. Results: Better VA, more polyp regression and less inj with combination tx (8.3 letters vs 5.1 letters, 69% vs 34%, median 4 vs 7) $ Novartis.


Efficacy and Safety of Intravitreal Aflibercept for Polypoidal Choroidal Vasculopathy (PLANET study), 2018  and  
n=318, 1y
Study question: Aflibercept q4w *3, then q4w with PDT or sham. Maintenance was q8w.
Results: AFB was non-inferior to combination with +10.7 letters vs 10.8. But had less polyp regression 38% vs 44.8% in the combination tx. Less polyps w/o leakage 81% vs 88% in the combination tx. Only 15% required PDT so no power to show benefit. $ Bayer.
