Study question: 3 VS 7 days face down position after macular hole surgery
Study results: hole closed successfully in 93.1% (27) in the 1-week group VS 87.5% (21) in the 3-day group - not statistically significant.
Enzymatic vitreolysis with ocriplasmin for vitreomacular traction and macular holes (MIVI-Trust), 2012 ⏩
n= 464 treated, 188 placebo. 28 days.
Study Question: Ocriplasmin (125 μg) VS placebo injection for symptomatic vitreomacular adhesion. Study results: Vitreomacular adhesion resolved in 26.5% of ocriplasmin vs 10.1% of placebo. Nonsurgical closure of macular holes was achieved in 40.6% of ocriplasmin group VS 10.6%. BCVA more likely to improve by a gain of at least three lines tx. Adverse events more common in ocriplasmin group 68.4% of VS 53.5%. Serious ocular adverse events was similar.

Comparison of face-down posturing with nonsupine posturing after macular hole surgery: a meta-analysis, 2019 ⏩
Study question: meta-analyses. postoperative face down position VS no position. Study results: Face down improved closure rate compared to no-position (OR = 1.828). A more significant benefit in large MHs (≥400 μm, OR = 4.361). No difference in the MH closure rate for small MHs (< 400 μm). ILM peeling for large MHs significantly increase the MH closure rate. No difference existed for small MHs. Combined cataract surgery might not influence the MH closure rate under any circumstance. Duration of face down position not addressed.

Study question: Ocriplasmin VS sham (not intraocular) for VMA.
Study results: VMA resolution at day 28 significantly higher in the ocriplasmin group (41.7%) compared with the sham group (6.2%), effect was maintained up to 2y. VMA resolution was higher in subgroups with focal VMA, presence of FTMH, absence of ERM, and phakic. 50% of the Ocriplasmin group had a ≥2-line improvement in BCVA compared with 39.1% of the sham group. Nonsurgical FTMH closure rate was 30.0% for the ocriplasmin group compared with 15.4% for the sham group. No new safety issues were reported.
